Beginning of the Year Tips from the School Nurse Michelle Nivens, RN
August 19th 2009
Kindergarten Students
• Please bring in a copy of your child’s immunization record and a copy of a physical completed after August 25th, 2008
Students in Grades K thru 5
* Please notify me if your child has an illness/health concern, for example, asthma, severe allergies requiring medication, diabetes, heart condition, seizures and all other medical conditions.
• Children requiring medication while at school, please see me and I will give you a medication authorization that needs to be signed by a Medical Doctor
• Vision Screening will be done on all students the weeks of September 7th and September 14th. If your child does not pass the vision screen, I will send a referral letter home recommending further evaluation from an Eye Doctor.

• If you do not have insurance for your child or are underinsured and would like assistance feel free to come and see me. I have information regarding Community Resources for Free medical care, immunizations etc.
