Parents, eligibility documentation will include any 2 of the following:         1.  power bill    2.  gas bill  3.  water bill  4.  lease agreement/mortgage statement  5.  Statement of domicile  6.  Hardship caregiver documentation         NO CELL PHONE, LAND LINE PHONE OR CABLE BILLS PLEASE State requires your athlete to be released by a physican to participate in sports.    physical form
